REQUEST TO FILL OUT MULTIPLE INTAKE FORMS

We would like to apologize if you have received an email request to fill out MULTIPLE INTAKE FORMS. We are working with Jane to figure out why people are being sent multiple forms and we apologize for this. For now, please only fill out the single intake form that best describes your appointment booking or you are welcome to complete the form when you arrive to your appointment.

1. How do I create my account?

2. once I’ve setup my account, how do I login to my account?

We have not enabled the “Online Booking” feature at this time. So for now you will want to follow this link to be able to sign into your account to make any changes or view your upcoming appointments.


Jane Data is encrypted using 256 bit encryption when sent between your device and our servers (in the same way as your banking information would be). Jane Data is stored on secure servers on proper data centres. Jane has 2 data centres both located in the Montreal area.


3. How is Jane Data secured and stored?


No! We do not save any credit card information within Jane which means that we do not store any credit card information for our clients.

5. DO I HAVE TO GIVE OUT MY EMAIL ADDRESS?

Absolutely not. Again providing us with your email address is to allow you to create an account which will allow you to receive appointment notifications, see your upcoming appointments as well as your appointment history, fill out intake forms as well as receive documents such as exercise programs shared by your therapist.


YES! The notifications are for your convenience and they are very easy to opt out of if you feel overwhelmed by receiving so many emails! You can either let us know in person and we can opt you out of this perk for you, or you can do so in your client account.
