The Carrie Smith “Get Active” Community Grant
Founder Roots.
Carrie Smith founded Kemptville Physiotherapy Centre in 1996 and has dedicated countless hours to both our incredible community and sport at all levels. The Carrie Smith “Get Active” Community Grant was established so that we can continue the tradition that reminds us of where we started.
The Details.
Applicants who demonstrate a commitment to better our community and who pursue their goals of becoming physically active will be eligible to receive the grant. We will select two recipients each year (Spring & Fall), each recipient will receive a $150 grant that will go to registration fees, specific equipment or just the gas to get to the venue! There is no age criteria for the grant. Each recipient will receive a certificate and cheque in the name of our grant.
The Application.
Please complete the application and send it to us at info@kemptvillephysio.com. All applicants who meet the criteria, will be entered and each winner will be chosen at random. Selections will be made in March & September.
